Bottom line, I guess, the strictly urinary equipment is so closely connected to the strictly reproductive equipment that a little crosstalk between the two systems can't be eliminated with certainty in every individual. I'm sure that the nervous systems are set up to avoid this, since mixing urine with reproductive fluids can't be good for reproductive success, but again, everybody is different, and the stimulation of sex may just be overriding her safeguard.
Have you tried bringing her to orgasm manually to see if that triggers urination?
Maybe the best you can do is take her out for walkies just before sack-time to ensure her bladder is as empty as she can get it.
